However, the size is entirely justifiable due to the industry-leading five-day battery life of the Active Guardian. This gives you the liberty of not having to charge the device every single day. The device uses AT&T cellular service and also incorporates GPS location tracking and Wi-Fi location technologies, making it the only portable medical alert system that has both.
Furthermore, the device is IPX5 waterproof, meaning it can be worn in the shower, and it can come equipped with automatic fall alert technology. With all of this included in a single device, we are willing to forgive its bulkiness, which was not an issue while we were wearing it for our Active Guardian review.
Ports, Slots, and Ease of Use
On the exterior of this white device, there’s a round, gray help button with Braille letters. Above it is the loudspeaker, while below it, we see dual microphones and four LED indicators. They are for the battery, call status, Wi-Fi, and GPS signal, which we found to be a bit distracting.
On the sides, there’s a SIM card slot, an inactive USB port, and an on/off switch. On the top, it has slots for the lanyard, which come included. Also included are a clip-on carry case and a charging cradle.
Additional Features and Accessories
Medical Guardian Fall Detection is one of the few additional features for this system. It costs an additional $10, but it’s one of the most important features you can get. The other accessory the company offers is a lockbox. It can be yours for free if you opt for the annual plan. Otherwise, it’s an additional $2 per month.
Moreover, GPS comes included, and you don’t need voice extension and wall-mounted buttons. However, you can’t get fire and smoke monitoring, a check-in service, or multiperson monitoring without doubling the monthly subscription Medical Guardian cost. Nevertheless, this applies to all other mobile medical alert systems.
Response Times and Audio Quality
We have tested the Active Guardian numerous times, and it never failed to connect us to the live agent. Response times were fairly quick, though not as fast as those of our top pick. Still, they were perfectly fine, averaging around 25 seconds, and always under half a minute.
The device provided clear and moderately loud audio. According to our experience with the company’s monitoring services, and based on Medical Guardian user reviews, its live agents are always friendly and professional. What’s more, they can provide help in 200 languages.
It takes the help agents 25 to 30 seconds to pinpoint your exact location. The best mobile medical alert service on our list is capable of identifying your location instantly. We were a little disappointed by this because the system uses dual location technologies. The device first tries to connect to a Wi-Fi hotspot, and if it can’t establish a local connection, it uses the GPS signal.
Another disappointment came, noted by other Medical Guardian reviews as well, from the fact that this company does not offer GPS mapping and activity tracking. That is, the ability to track down med-alert users from home doesn’t exist.
Medical Guardian also doesn’t have a mobile app for this specific device. Seeing as most rival systems, such as the Bay Alarm Medical GPS alert system, offer these features, Medical Guardian has stated that it will add them soon. However, we must note that the company does have an app that can link up to its other systems.

Unlike the other mobile systems on our list, there is no device fee. So the Active Guardian plus the medical alert monthly cost comes down to around $45—twice the monthly cost of our top pick, which is a lot.
